WL#3552: Finish 4.1 support and ship plugin 1.0

Affects: Connector/.NET-5.2   —   Status: Complete   —   Priority: Medium 

The plugin as delivered from the contractor does not support MySQL 4.1.  The 
support is there but is buggy.  This worklog is to track fixing this support 
and shipping version 1.0 of the plugin.
